Terms & conditions for entry to the Eyebright Awards

  1. Information on how to enter/nominate and prizes forms part of these Conditions of Entry. Submitting an entry into the Eyebrights Awards is deemed to be an acceptance of these Conditions of Entry.
  2. Acceptance of entry forms commences on 3 June 2011. Entries close at 23:59:59 NZST on 10 August 2011. It is at the judges’ discretion whether or not any entry received after the closing date may be accepted for consideration.
  3. Entry is only open to Plunket teams made up of Plunket staff, volunteers or a combination of both.
  4. For a chance to win, entrants must submit a fully completed printed entry form or fill in an online form by the closing date from http://www.plunket.org.nz/what-we-do/who-we-are/eyebright-awards and sent to PO Box 5474, Wellington
  5. Nominations are deemed to be received at the time of receipt by Plunket National Office.
  6. A grant of up to $1500 will be awarded to each of the two winning teams and will be at the discretion of the judges and the sponsors and subject to specific criteria including, but not limited to, the grant being for the benefit of Plunket and children and families in the local community.
  7. The two grants, each of up to $1500, cannot be transferred, exchanged or used for any purpose except that for which the grant is made and must be allocated within Plunket’s current financial year (i.e. by the end of June 2012)
  8. Huggies and Wattie’s (“ The sponsors”) and Plunket National Office will not be liable for any loss or damage whatsoever which is suffered (including but not limited to indirect or consequential loss) or for any personal injury suffered or sustained as a result of receiving, taking or using any prize except for any liability which cannot be excluded by law.
  9. The winning teams will be advised via telephone and winning teams will be required to adhere to an embargo prior to official announcement.
  10. Teams that submit nominations, but do not win, may be required to participate in promotional activity.
  11. The judges’ decision is final, and no correspondence will be entered into, including in the event of a dispute.
  12. Plunket National Office, the sponsors and the judges accept no responsibility for lost, late, stolen, damaged or misdirected entries.
  13. Upon acceptance of the grant, the project and the winning teams may be required to participate in further media/promotional activity.
  14. All entries become the property of Plunket National Office and may be included in newsletters, media releases and other promotion of the sponsors and the Eyebright Awards, provided that where any entry contains personal information, such use must be subject to the consent of the person or persons concerned.
  15. Any tax liability arising as a result of accepting any prize is the responsibility of the winning teams.
  16. Any personal information submitted with an entry will be held by the Plunket National Office in accordance with the provisions of the Privacy Act 1993. Any amendment to such personal information should be addressed to that office.
