Applications for senior nurse level: These applications are accepted at any time throughout the year. You can contact a Plunket PDRP assessor to arrange the portfolio assessment, or you can ask the PDRP coordinator to arrange an assessor for you. Once the assessor is arranged, you will need to email the completed application form through to the PDRP coordinator. Then, when the portfolio is ready to be assessed, the portfolio is sent via courier (track and trace with signature) directly to the PDRP assessor (check that you have the correct name, role and courier address).
Below are the new forms from 1 August 2019. If you have commenced your portfolio already the previous forms can still be used. Please see PDRP Forms prior to 31.07.2019.
